Policy - Immunization: Exclusion and Exemption from School Attendance

NEW REQUIREMENTS:

Physical exams are required for:
Pre-K/Early Childhood Students
Kindergarten Students
5th Grade Students
9th Grade Students (informational letter)
All students new to District 428

Within one year prior to entering preschool, upon the first day of attendance for kindergarten or the first grade and upon entering the fifth and ninth grades, students shall present proof of having undergone a health examination by a medical doctor, advanced practice nurse, or physician assistant.. Transfer students entering the school district for the first time, regardless of grade, shall present such proof within thirty days of their enrollment. Students must also present proof of having received the immunizations against preventable communicable diseases as required by the rules and regulations of the Illinois Department of Public Health.

If a student does not comply with the requirements of this policy, such student shall be excluded from school until proof of having received the required health examination and immunizations is presented to the district. All new students who are first time registrants shall have thirty (30) days following registration to comply with the health examination and immunization regulations. During a student's exclusion from school for non-compliance with this policy, the student's parents shall be considered in violation of section 26-1 of The School Code of Illinois and subject to any penalty imposed by section 26-10 of The School Code of Illinois. A student may be exempted from the requirements of this policy for religious or medical grounds. If a student's parents or legal guardian objects to the health examination and immunizations on religious grounds, then such parents or legal guardian must present to the Superintendent a signed statement of objection, detailing the grounds for such objection. If a student is exempted from immunizations on medical grounds, then the examining physician shall endorse such fact upon the health examination form.
